Credentialed Alcoholism and Substance Abuse Counselor
Training Declaration
Introduction:
The practice of a Credentialed Alcoholism and Substance Abuse Counselor consists of four (4)) core functions, (1) Knowledge in Substance Abuse disorders; (2) Substance Abuse Counseling; (3) Assessment, Clinical Evaluation, Treatment planning, Case Management, Patient, Family and Community Education; (4) Professional and Ethical Responsibilities.
To be fully credentialed by New York State Office of Addiction Services and Support, the candidate must complete the five (5) phases as describe below, including obtaining a passing score on the IC&RC ADC examination. Vertex Training Center will provide various courses that will meet the prerequisites for the following NYS Certified Addiction Professional credential:
Complete Part A-Summary Form
Submit social security number
Sign & date Part A form
Review & Sign Cannon of Ethical Principles, Misconduct Attestation, NYS Justice Center
Code of Conduct
Submit application fee to NYS OASAS

Complete the Evaluation of Competency & Ethical Conduct Form
Complete & sign Applicant Consent to Release Information Form
Submit 2 evaluations forms with corresponding supporting documents

You will be required to fill out an application before purchasing courses. There is an application fee of $35.
Complete “Applicant Consent to Release Information Form” and Work Experience
Verification Record Form.
Submit corresponding documents from paid employer HR office or Clinical Supervisor (volunteer/internship)

Education & Training Experience (Part D)
Complete and obtain a Certificate of Completion for the 350 hours standardize curriculum and/or academic transcripts.
Section I: Knowledge of Substance Abuse Disorders (85 hrs)
Section II: Alcoholism and Substance Abuse Counseling (150 hrs)
Section III: Assessment, Clinical Evaluation, Treatment Planning, Case
Management and Patient, Family and Community Education (70 hrs)
Section IV: Professional and Ethical Responsibilities (45 hrs)
High school diploma or equivalency,
Associate Degree, may be substituted for a maximum of 1000 hours of work experience
Bachelor’s Degree, may be substituted for a maximum of 2,000 hours of work experience
Advanced Degree* {Master}, may be substituted for a maximum of 4,000 hours of work experience
350 clock hours of approved education/training
Passing Score on the IC&RC ADC Examination

Procedures
Submit application fee as per the specific requirement, based on residency.
Student will meet with Vertex Training Center staff to be assessed for financial services and/or referred to following once eligibility criteria are established:
* City or State financial aid via NYS-DOL: ACCESS-VR, or any other grant or aid.
Employee Reimbursement Program
* Student working and are eligible for financial reimbursement by their current employer must submit Employee documentation and signed contract with Employer for direct or partial payment to Vertex Training Center
